Resident Financial Assistance
Eligibility of the Applicant:
Must be a current resident in good standing for at least:
- Independent Living Entrance Fee Residents: two years
- Healthcare Residents: three years
- Independent Living Renter Resident: four years
- Must have demonstated financial need as determined by the Foundation Board.
- Source of income / assets have incurred substantial interrruption prior to temporary Living Well Foundation assistance.
Residents should apply for financial assistance at least three months prior to exhausting available assets and resources. Contact the Community Business Office, email info@Living-Well-Foundation.com or call 704-815-7370 for a Financial Assistance Application Packet. Complete and return the application with supporting documentation to the Executive Director/Business Manager/Business Office.
To protect the privacy and confidentiality of residents, applications for financial support, together with supporting documentation should be submitted to the Business Office of the Community. The Executive Director will perform a review of each application, make an initial recommendation and forward the application to the Foundation Board for final review and consideration.
Applications will be received and reviewed by the Foundation Board on a quarterly basis and must be received 30 days prior to quarter end. When received, the applicant will be notified that the application has been received via email or in writing. The Foundation Board reserves the right to request additional financial information from the resident applicant or family members of the applicant.
Upon completion of the review by the Foundation Board, the applicant will be notified of the decision in writing. Approved applications are for a period of up to one year.
